9/26/2023 EnchantaVaughn Approved Behavior: Client seemed very excited to see his counselor and initiated conversation right away. Client was seen being polite and respectful to his mom who left the room to give client some privacy. Client talked a lot about his day at school and how this year was much more fun and interesting. Client appeared to be in a cheerful mood and displaying good behaviors in the home. Client cooperated with counselor communicating effectively. Behavior: Client was observed making insightful comments and engaging in family discussion. Client demonstrated excellent mannerisms. Client was focused, paying attention, and on task. Client was observed showing good leadership skills as evidenced by his courage and sportsmanship with speaking in front of others. Client expresses himself in an assertive but confident way when it comes to his work and was supportive to mom and siblings. Client continues to execute responsibility for his actions by making good choices and utilizing all mental health skills. Intervention: QMHP processed with client about his day. QMHP prompted client to share anticipated thoughts and feelings and to demonstrate how this process reduces angry reactions at home, at school, and in the community. QMHP praised client for his positive behaviors and verbalized to client that he is doing an amazing job with following through with orders and maintaining a good attitude. QMHP praised client for his efforts and being respectful to his mom. QMHP supported client making good decisions and having positive behaviors in the home. Intervention: QMHP supported client involvement in selected activities. QMHP asked appropriate questions regarding activity. QMHP processed with client about what he learned from the activity. QMHP helped client develop healthy self-esteem by reducing level of anxiety and emotional reactions. QMHP monitored effects of meditation and coping practices daily and documented his progress in his treatment chart. QMHP assisted client with managing anger and frustration by reviewing and role-playing effective coping skills. Response: Client demonstrated appropriate interpersonal skills, assertiveness, and confidence in self. Client was able to speak calmly and demonstrate his improved verbalization skills while conversing with mom. Client recognized his triggers and was able to identify similarities and differences in his behaviors and attitude that reflect on certain situations that he’s constantly involved in. Client expressed excitement when it was time to demonstrate his strengths in front of authority because he was confident and prepared. Response: Client actively participated with QMHP as they discussed and processed his current behaviors. Client did not display any outbursts at this time. Client appeared very relaxed and upbeat during exercise and needed no redirection. Client retained a lot of the information he was taught which motivates him to pay more attention to the things that keep him focused and uplifted. Client said he was going to practice coping strategies and relaxation techniques in all aspects of his daily living routine.
